Practical recommendations for patients at high risk of radial crossover (MATRIX Score ≥41):
- Counsel patients on the risk of radial failure during the informed consent process;
- Consider the use of ultrasound-guided radial artery cannulation;
- Select materials/techniques that can facilitate vascular navigation (i.e., hydrophilic sheaths, guidewires, and catheters, dedicated radial shapes);
- Examine femoral arteries to plan safe alternative access if radial fails;
- If possible, schedule the procedure when expert radial operators are available.
